Seattle Appointments Only

If you are scheduling an appointment at my Seattle (Fremont) office please understand that for me to come to Seattle (from Ashland, Oregon) it is a 16 hour round trip drive. I have committed to coming to Seattle every 2 to 3 months, but in order for this arrangement to continue I will also need your commitment to this time. Therefore, it is essential you understand that the payment you make is non-refundable and non-transferrable to another date. So, if you are unable to make your appointment (for any reason) I will not be able to provide you with a refund.

How to Cancel or Reschedule an Appointment

If you need to cancel or reschedule, please be aware that failure to request via email to reschedule or cancel your first appointment 4 days in advance results in a forfeiture of the prepayment, which will be used to cover the “missed appointment fee”.

You MUST cancel your appointment at least 4 days prior to the appointment, in order to receive a refund minus $25 to cover processing fees.

If you need to cancel or reschedule you must do so via email using the HIPPA compliant email ([email protected]). DO NOT leave a voice mail or text message, as I am less likely to see these forms of communication.
