Naturopathic Medicine Telehealth Consultation (Initial Consultation)

Description
During your first telehealth (Zoom) visit we will do the following:
- Naturopathic intake
- Nutritional evaluation
- Lab recommendations
This appointment will last about 1 hour.

The following are the steps for booking an appointment:
- Select the type of appointment you would like.
- You will then be asked to input some information. Once you submit your request an email will be sent to me and I will respond with possible dates and times.
- You WILL NOT be charged at this time.
- After we have settled on a date and time I will send the invoice to you. In order to confirm this appointment you will need to pay for the appointment at this time.
- If you would like to reschedule to a different day you will need to notify me 4 days prior to your confirmed appointment date. If you need to cancel your appointment and would like a refund, you will need to cancel this appointment via email at least 4 days in advance. At that time will be given a refund of $425 (the pre-payment minus a $25 processing fee). Failure to change or cancel 4 days in advance results in a forfeiture of the full $450.00, which will be used to cover the “missed appointment fee”.
